Semaglutide at a glance
Semaglutide is a long-acting GLP-1 analogue that enhances glucose-dependent insulin secretion, suppresses glucagon, slows gastric emptying and reduces appetite. Its evidence base consists of large phase 3 randomized controlled trials. In STEP 1 (1,961 adults with overweight/obesity, no diabetes), once-weekly semaglutide 2.4 mg produced a mean body-weight change of -14.9% vs -2.4% with placebo at week 68 (Wilding et al., NEJM 2021, PMID 33567185). In SUSTAIN-6 (3,297 patients with type 2 diabetes at high cardiovascular risk), semaglutide significantly lowered the rate of cardiovascular death, nonfatal myocardial infarction or nonfatal stroke versus placebo over 104 weeks (Marso et al., NEJM 2016, PMID 27633186). The most common adverse effects are gastrointestinal (nausea, vomiting, diarrhea, constipation). Labeling carries a boxed warning for risk of thyroid C-cell tumors based on rodent data and is contraindicated in personal or family history of medullary thyroid carcinoma or MEN 2, with additional warnings for pancreatitis, gallbladder disease, diabetic retinopathy complications, acute kidney injury, and hypoglycemia when combined with insulin or sulfonylureas.

- What does the research on Semaglutide show?
- Semaglutide is a long-acting GLP-1 analogue that enhances glucose-dependent insulin secretion, suppresses glucagon, slows gastric emptying and reduces appetite. Its evidence base consists of large phase 3 randomized controlled trials. In STEP 1 (1,961 adults with overweight/obesity, no diabetes), once-weekly semaglutide 2.4 mg produced a mean body-weight change of -14.9% vs -2.4% with placebo at week 68 (Wilding et al., NEJM 2021, PMID 33567185). In SUSTAIN-6 (3,297 patients with type 2 diabetes at high cardiovascular risk), semaglutide significantly lowered the rate of cardiovascular death, nonfatal myocardial infarction or nonfatal stroke versus placebo over 104 weeks (Marso et al., NEJM 2016, PMID 27633186). The most common adverse effects are gastrointestinal (nausea, vomiting, diarrhea, constipation). Labeling carries a boxed warning for risk of thyroid C-cell tumors based on rodent data and is contraindicated in personal or family history of medullary thyroid carcinoma or MEN 2, with additional warnings for pancreatitis, gallbladder disease, diabetic retinopathy complications, acute kidney injury, and hypoglycemia when combined with insulin or sulfonylureas.
