Tirzepatide at a glance
Tirzepatide activates both the GIP and GLP-1 receptors, improving insulin secretion and sensitivity, lowering glucagon, slowing gastric emptying and reducing appetite. Its evidence base consists of large phase 3 randomized controlled trials (the SURMOUNT and SURPASS programs) that supported the FDA approvals for type 2 diabetes and chronic weight management. In SURMOUNT-1 (2,539 adults with obesity, without diabetes), once-weekly tirzepatide produced mean weight reductions of approximately 15% to 21% across the 5/10/15 mg doses versus -3.1% with placebo at week 72 (Jastreboff et al., NEJM 2022, PMID 35658024). In the head-to-head SURPASS-2 trial (1,879 patients with type 2 diabetes), tirzepatide was noninferior and superior to semaglutide 1 mg for HbA1c reduction and produced greater weight loss at 40 weeks (Frias et al., NEJM 2021, PMID 34170647). The most common adverse effects are gastrointestinal (nausea, diarrhea, vomiting, constipation). Labeling carries a boxed warning for thyroid C-cell tumor risk based on rodent data and is contraindicated in personal or family history of medullary thyroid carcinoma or MEN 2, with additional warnings for pancreatitis, gallbladder disease, hypoglycemia when combined with insulin or secretagogues, acute kidney injury, hypersensitivity reactions and diabetic retinopathy complications.

- Tirzepatide activates both the GIP and GLP-1 receptors, improving insulin secretion and sensitivity, lowering glucagon, slowing gastric emptying and reducing appetite. Its evidence base consists of large phase 3 randomized controlled trials (the SURMOUNT and SURPASS programs) that supported the FDA approvals for type 2 diabetes and chronic weight management. In SURMOUNT-1 (2,539 adults with obesity, without diabetes), once-weekly tirzepatide produced mean weight reductions of approximately 15% to 21% across the 5/10/15 mg doses versus -3.1% with placebo at week 72 (Jastreboff et al., NEJM 2022, PMID 35658024). In the head-to-head SURPASS-2 trial (1,879 patients with type 2 diabetes), tirzepatide was noninferior and superior to semaglutide 1 mg for HbA1c reduction and produced greater weight loss at 40 weeks (Frias et al., NEJM 2021, PMID 34170647). The most common adverse effects are gastrointestinal (nausea, diarrhea, vomiting, constipation). Labeling carries a boxed warning for thyroid C-cell tumor risk based on rodent data and is contraindicated in personal or family history of medullary thyroid carcinoma or MEN 2, with additional warnings for pancreatitis, gallbladder disease, hypoglycemia when combined with insulin or secretagogues, acute kidney injury, hypersensitivity reactions and diabetic retinopathy complications.
